Super Bowl Betting Questionnaire

Hey all, couple years ago I attended a Superbowl party where we filled out a questionnaire with about 20 Superbowl specific questions ranging from Coin toss and National anthem length to First INT and Touchdown.

Anyone know where I can find one of these online?

Only thing I could find online was the list of the props bets.

I can just think of some things you can do, from previous yrs of what I’ve done.

Coin Toss (Heads/Tails) or Seahawks/Patriots
National Anthem length (over/under 2minutes or whatever)
First to:
Have their teams cheerleaders shown
Score a TD
Score a Run TD
Score a throw TD
Have a turnover
Make a field goal
Have a 3-and-out

Then a combined score. If you are within 2 pts you get the point:
1^st Quarter – if the score is 10-7, combined score of 17 (if you guess 15 you get this box right)
2^nd quarter – do running score, easier to follow. 17-7 after 2 combined score is 24, guess 26 you get it right
3^rd quarter
4^th quarter
