Erik Karlsson (lower body) is expected to play in Game 5, per Senators coach Guy Boucher.
Karlsson was shaken up towards the end of the second period of Thursday's contest and left the game as a result. Boucher said afterwards that Karlsson could have returned if needed, so it's not surprising that he'll be available on Saturday. May 5 - 1:28 PM

I think they like to avoid surgery where possible... Four months is a long time and you have to think eats into training camp so what impact does that have if he is a few weeks behind and he doesn't catch up initially? Hope he makes a full recovery in time for the season - unless you don't own him and pencil in a potential slow start and get in early to try and acquire him....

Yes, surgery - this is the real problem which can add couple of months to recovery from injury
